Keto vs Low Carb: Where the Line Is and Whether It Matters
Quick answer: Low carb is a range; keto is a threshold inside it. Below a certain carb intake your body makes enough ketones to count as ketosis, and that's keto. Above it, you can still be eating far fewer carbs than average and getting many of the same benefits. KetoDick's own beginner rule is under 50 g of total carbs a day, and plenty of people get what they want there without chasing ketone numbers.

The carb ladder
Start from where most people are. KetoDick gives the standard American diet as about 250 g of carbs a day. From there he lays out a ladder: under 100 g is what most scientists would call very low carb, under 50 g extremely low, and under 20 g is where ketosis is most reliable (video DoSUgIr2rNo).
His point is that every rung is already a dietary intervention. Cut from 250 g to 150 g and you've changed your diet a lot. Cut to 100 g and you're well on your way (video DoSUgIr2rNo).
Back-of-envelope, with his own figure: 250 g is about 1,000 calories of carbohydrate a day (video DoSUgIr2rNo), so about 4 calories a gram. Getting to 50 g removes about 800 calories' worth of carbs. Where those calories go next, into fat and protein or just gone, is the weight-loss question.
Keto vs low carb side by side
| Low carb | Keto | |
|---|---|---|
| Defined by | A carb range, below the average diet | A threshold: ketones in the nutritional-ketosis range |
| Typical carb level in his videos | Under 150, under 100 | Under 50 as the usual definition; 20 as the common target (video ffwBPoKxlDI) |
| Need to test? | No | Optional; strips are cheap biofeedback |
| Adaptation period | Milder | Weeks of fat adaptation, often a rough patch early |
| Room for vegetables and some fruit | More | Less, so spend carbs on greens |
| Main benefit he cites | Much of keto's benefit, per the literature (video qfy0IBkPF4A) | Hunger control and steady energy once adapted |
Where the 20 g line comes from
The 20 g number is everywhere, and KetoDick questions it. He asks where the exact figure comes from when it's applied the same way to a 200-plus-pound active man and a 90 lb woman, and reads 20 g as a study protocol rather than a law (video 1vyxuWlxT5M).
His own numbers have moved. In 2017 he gives under 20 g as the start, with some people tolerating up to 50 (video s_sNd1v8Low), and says under 50 g a day is the usual definition while 20 g is the colloquial recommendation because some people need it (video ffwBPoKxlDI). By the 2018 basics video his rule for beginners is simply under 50 g of total carbs, not net (video UygKGoP_BtA).
He counts total because US labels don't split soluble from insoluble fiber, which makes net carbs a guess. More in why 20 grams of carbs.
Is ketosis the goal?
Here's the turn that surprises people. In his 2019 rebuttal of a Men's Health article, KetoDick says ketosis isn't the goal. Low carb is the goal, and ketosis is a perk for energy and hunger (video 1JbK1_ClDmY).
That lines up with something he said in 2017: "just eat less carbs." He cites studies where low-carb diets under 150 g still worked (video xyEZL0HuejI). And in his tapering video he answers the "that's not keto, it's low carb" objection directly: low-carb diets have a lot of medical and scientific literature showing many of keto's benefits, so if you have health concerns and keto feels extreme, a general low-carb diet is worth considering (video qfy0IBkPF4A).
So "is this keto?" is often the wrong question. "Is this working for me?" is the right one.
How to choose your number
- Name your goal. Weight loss, blood sugar, maintenance, or just eating better. Different goals tolerate different numbers.
- Start where you can stick. If 20 g feels impossible, a taper from 150 to 100 to 50 is his suggested on-ramp (video DoSUgIr2rNo).
- Try 50 g total as a default. It's his simplest rule, and it leaves room for vegetables (video UygKGoP_BtA).
- Judge by hunger and measurements, not ketone colour. If you're not hungry and your tape measure is moving, the number is working.
- Go lower only if you need to. If you're stalled after the first couple of months and hunger is back, test a lower number for a few weeks.
What the research says
- The American Diabetes Association's consensus report says cutting total carbohydrate has the most evidence for improving blood glucose, with low-carb patterns one option among several (Evert et al., 2019). Its 2025 Standards of Care accept several eating patterns, low-carb among them, when individualised (ADA, 2025).
- Diabetes UK says low-carb diets can be effective short-term for weight and glycaemic control, with no single ideal macronutrient ratio (Diabetes UK, 2018).
- DIETFITS's healthy low-carb arm was moderate, not ketogenic, and lost about as much as healthy low-fat at 12 months (6.0 vs 5.3 kg) (Gardner et al., 2018).
- Very-low-carb ketogenic diets came out 0.91 kg ahead of low-fat in a pooled analysis of 13 trials of 12 months or more (Bueno et al., 2013).
- In a non-randomised study of people with type 2 diabetes, a carbohydrate-restriction intervention lowered HbA1c from 7.6% to 6.3%, with about 12% weight loss and less diabetes medication (Hallberg et al., 2018).
The lipid association's summary fits both: not superior for long-term weight loss, possibly more short-term loss, better triglycerides and HDL, variable LDL (Kirkpatrick et al., 2019). The evidence pages here don't give a trial that pits 50 g against 20 g directly.
KetoDick's take
I've said that if you're confused, just eat less than 50 carbs a day and nothing else matters, ideally 20 (video xyEZL0HuejI). I've also said ketosis is a perk, not the goal (video 1JbK1_ClDmY).
If you take insulin or other glucose-lowering medication, going from 250 g to 50 g of carbs is a big change in what those drugs are working against. Plan the step-down, and any dose changes, with your doctor (video UygKGoP_BtA).
Key takeaways
- Low carb is a range below the average diet; keto is the threshold where ketosis kicks in.
- His ladder: about 250 g typical, under 100 very low, under 50 extremely low, under 20 most reliably ketogenic.
- His simplest beginner rule is under 50 g total carbs, not net.
- Ketosis is a perk; low carb is the goal. Judge by hunger and measurements.
- On diabetes medication, plan any carb cut with your doctor.
