Keto made easy

The Week-Three Keto Stall: Why It Happens

Quick answer: The week-three stall is expected. In week one you lose water as your glycogen goes. By weeks two to four the water is gone, but your body hasn't finished adapting to run on fat, so the scale sits still and you feel flat. KetoDick calls weeks two and three "a slog," from the video, and says most people plateau there (video uDw65ERtsA0). It's a phase, not a sign keto has stopped working.

The shape of the first month

KetoDick pre-recorded his week-by-week schedule so people would know their experience is normal (video uDw65ERtsA0). Here it is in short:

Week What usually happens, in his schedule
1 Water weight drops as glycogen goes: about 3 lb, or 7–10 lb for heavier people
2 Digestive upset as your gut adjusts to fewer carbs
2–3 Low energy, not yet fat adapted; most people plateau
3–6 onward Normal fat loss starts, about 0.5–3 lb a week depending on size

Read that table and the stall stops being mysterious. A big drop, then nothing, then slow and steady. People see great losses in week one and then nothing in weeks two and three, as he puts it. If nobody told you, it feels like keto worked for seven days and quit.

Why the scale stops in weeks two to four

Two things are happening at once.

The easy water is gone. Glycogen is stored with roughly twice its weight in water (video uDw65ERtsA0). In his 2018 figures that's about 700 grams of glycogen plus 1,400 grams of water, around 4.4 pounds (video UygKGoP_BtA). Once that's drained, you can't lose it again. So the week-one rate was never going to last.

Fat adaptation isn't finished. He describes the liver "up-cycling" its enzymes and the mitochondria adjusting to ketones, which takes at least two to three weeks (video cTGYIFOTPl8). His timelines for full adaptation vary by video: about 2 to 4 weeks for thinner people and 4 to 8 for heavier ones in one (video NUJNFG6lLWw). Until then, glycogen is gone but your body can't yet draw on its own fat well, so the energy gap and the hunger that should shrink it aren't fully working yet.

Back-of-envelope: week one might show 3 to 10 pounds, mostly water. Real fat loss in his schedule runs from half a pound to three pounds a week. Going from 7 pounds in a week to half a pound feels like a stall, even when it isn't one.

Why you feel worse at the same time

The stall usually arrives with the worst of the transition: low energy, poor sleep, maybe cramps or headaches. This is the stretch people call keto flu.

KetoDick links the gut upset in week two to the gut microbiome adjusting to fewer carbs (video uDw65ERtsA0). In his beginner video he lists gas, bloating and constipation, and points to fibre from vegetables as one reason to keep eating them (video UygKGoP_BtA). See keto and your gut microbiome.

The fatigue, in his model, is the gap between glycogen running out and fat adaptation kicking in (video 9r2Y74AnjUQ). Cramps he puts down to electrolytes, a separate problem with a separate fix (video DoSUgIr2rNo); the guide's keto flu section covers it. If you take blood pressure medication or have kidney disease, talk to your doctor before you start adding sodium or potassium.

How to get through it

So what you do is ride it out on purpose, rather than "fixing" it:

  1. Stay off the scale. He says skip it for the first 30 days, then weigh weekly or every two weeks (video uDw65ERtsA0).
  2. Don't cut calories in month one. Eat until you're full, keep carbs low and get your protein (video 7Aixw4uRksI). Telling month-one people to cut fat is, from the video, "kind of cruel" (video NUJNFG6lLWw).
  3. Keep the vegetables. Fibre helps with the gut side of the transition (video UygKGoP_BtA).
  4. Measure something else. Take photos from day one and use a tape or string (video UygKGoP_BtA). See losing inches, not weight.
  5. Use the weeks to learn. He treats the first 30 days as time to learn labels and recipes, not as a painful diet (video 7Aixw4uRksI).
  6. Give it the full 60 days before you decide whether keto works for you (video uDw65ERtsA0).

If you're well past week six and still stuck, that's a different problem. Start with why am I not losing weight on keto?

What the research says

The evidence pages don't include a trial that tracks week-by-week scale weight on keto, so his schedule rests on his own experience and reading. A few findings fit the pattern.

The rough patch is early and passes. In an analysis of online posts, reported "keto flu" symptoms clustered in the first weeks and were mostly described as transient (Bostock et al. 2020).

Adaptation takes weeks. In a classic fasting study, after weeks without food, ketones replaced glucose as the brain's main fuel (Owen et al. 1967). That's fasting, not a keto diet, but it shows the switch is gradual. Ketone production itself is set by insulin and glucagon in the liver (StatPearls, Biochemistry, Ketogenesis).

Early weeks aren't a fat-loss fast lane. In a metabolic ward study of 17 men switched to a ketogenic diet at the same calories, body-fat loss slowed after the switch, even though energy expenditure rose by about 57 calories a day (Hall et al. 2016). That doesn't explain your scale, and it was a small, short study, but it fits the idea that the first weeks on keto are a transition rather than a burst.

KetoDick's take

His most direct line on this comes from his longest early video: you're not really on the diet until week three, four or five, from the video, and how you feel on day ten is irrelevant (video cTGYIFOTPl8). In the same video he puts the start of real fat loss at days 30 to 60.

He also admits the timing varies. Adaptation speed depends on sex, age, where you are in your menstrual cycle and more, he says (video NUJNFG6lLWw). So if your stall runs into week four or five, that's still inside his range. The point of his schedule is that you know it's coming, so you don't quit in the one stretch that was always going to be hard.
