Keto made easy

Is Keto Dangerous? An Honest Answer

Quick answer: Keto isn't harmless and it isn't poison. It's an intervention, and like any intervention it carries risks that depend on who you are. KetoDick compares it to surgery: risky, sometimes worth it, and not necessarily permanent (video 1m1hPgaRIg4). The standard high-carb diet has risks too. The way to find out what keto is doing to you is blood work and a doctor, not a forum.

Why "is it dangerous?" is the wrong shape of question

Announce you're starting keto and someone will message you, worried. KetoDick acts this out in one of his earliest videos, then points out that nobody warned you when you were heavy (video 1m1hPgaRIg4).

His answer is to compare risks instead of asking "safe or not." Almost every medical intervention carries risk, he says; you choose one for a reason (video 1m1hPgaRIg4). And he asks whether eating around 250 g of carbs a day, the pattern he links to type 2 diabetes and heart disease, isn't also dangerous (video 1m1hPgaRIg4). Nobody sends warnings about french fries.

So the better question is: compared with how I eat now, and given my health, what are the risks and how will I watch for them?

The real risks worth knowing

These are the ones that come up in his videos and on the evidence pages:

Risk Who it matters most for What to do Source
Medication doses becoming too strong People on diabetes or blood pressure medication Review doses with your doctor before starting video UygKGoP_BtA; Hallberg 2018 below
LDL cholesterol rising Some people; varies a lot Check lipids before and a few months in NLA 2019, Bueno 2013 below; videos 1m1hPgaRIg4, cTGYIFOTPl8
Electrolyte problems (cramps, palpitations, fatigue) Everyone early on; more so with kidney issues Salt and broth; doctor before potassium or magnesium supplements videos lbRPlGZ3Rvw, -dHjt_Bpruk
Kidney stones, heart or liver concerns People with history or genetic risk Get it checked rather than dismissing it video uXzscHudgt0
Thin nutrition from leaning on fat Anyone living on fat bombs and butter coffee Vegetables, protein, a multivitamin videos uXzscHudgt0, UygKGoP_BtA
Disordered eating Anyone with an eating-disorder history Involve a doctor or therapist first see eating disorders

On cholesterol specifically, he says he has read that about one in four people on low-carb high-fat diets develop a blood-lipid problem, and tells people to get lipids checked (video cTGYIFOTPl8). That figure is his recollection; it isn't on the evidence pages. In his later basics video, his reading is that cholesterol may spike early and improve over 6 to 12 months (video UygKGoP_BtA). The literature below is more mixed than that on LDL.

Ketosis is not ketoacidosis

This one deserves its own section, because it's behind a lot of the fear. Nutritional ketosis, what a keto diet produces, and diabetic ketoacidosis (DKA), a medical emergency, both involve ketones, but they are different states. The NIH's clinical overview of the ketogenic diet describes how the two differ (Masood et al., StatPearls).

The people who need to take this seriously are those with diabetes: anyone with type 1, or anyone on diabetes medication, should not run this experiment without their doctor. KetoDick likewise says that with diabetes, pre-diabetes or any kidney issue, a very dark ketone strip is a reason to talk to your doctor (video GojQvo_2GS0). See ketoacidosis.

How to find out if it's safe for you

So what you do:

  1. Get baseline labs before you start. He recommends this, especially if you're older or have health history (video GojQvo_2GS0). Lipids, glucose or A1c, kidney function and electrolytes are a sensible ask.
  2. Tell your doctor what you're doing, especially about any medication (video UygKGoP_BtA).
  3. Recheck after a few months. He planned to check his own lipids, noting that for some people lipids become a problem around six months in, and he checked his blood sugar every morning (video 1m1hPgaRIg4).
  4. Follow up symptoms with a doctor. Cramping, sleep trouble or sweats: his advice is don't just ask internet forums (video 1m1hPgaRIg4).
  5. Eat real food, not just fat. Leafy greens, herbs, protein; go easy on fat bombs and bulletproof coffee (video uXzscHudgt0).
  6. Be willing to stop. If your numbers go the wrong way, that's information, not failure.

What the research says

KetoDick's take

His honest worry isn't the food. It's confirmation bias: a keto forum will answer every question with keto, and keto may not suit everyone's genetics or gut (video 1m1hPgaRIg4). He also notes some forum weight loss may come from simply paying attention to food (video 1m1hPgaRIg4).

Where he and the literature line up: check your blood work and don't assume. Where they differ a little: he summarizes long-term studies as finding keto generally well tolerated with cholesterol improving over time (video UygKGoP_BtA), while the lipid evidence above says LDL can go up and varies by person. Both point to the same action. Test, don't guess.

When to involve your doctor

Before starting if you take diabetes medication (especially insulin), blood pressure medication, or anything with a dose tied to blood sugar or fluids. If you have kidney disease, a history of kidney stones, or heart disease. If you're pregnant or breastfeeding. If you have an eating-disorder history. And any time you have palpitations, chest pain, severe cramps or symptoms that don't settle after the first few weeks.
